How to use the downloaded form:
- Download the file and view it in Adobe Acrobat Reader.
- Print both pages of the form.
- Complete all appropriate spaces with the names and information for each person's pass.
- Calculate the total amount due.
- Sign on page two.
- Enclose a check with payment (made out to Squaw Valley Ski Corporation). Or, you may write your Visa or Mastercard number on the form and we will process it for payment.
- Mail both pages of the form and payment to:
